Vaginal Dryness and Painful Sex

A common and very treatable problem that is rarely raised, either by patients or by doctors. Consultant assessment and treatment options explained properly.

Around half of women notice vaginal dryness after the menopause, and most never mention it. It is not something you have to accept, and it does not usually need anything complicated to treat.

What to expect

Assessment and a treatment plan in one visit

What is actually happening

Falling oestrogen thins and dries the vaginal tissue. This is genitourinary syndrome of the menopause, and it affects the bladder and vulva as well.

The symptoms it causes

Dryness, burning, itching, discomfort or pain during sex, bleeding afterwards, and recurrent urinary symptoms that keep being treated as infection.

Examination

A gentle look at the vulva and vagina to confirm the cause and rule out skin conditions such as lichen sclerosus, which needs different treatment.

Local oestrogen

Vaginal oestrogen is highly effective, acts locally rather than throughout the body, and is suitable for many women who cannot or prefer not to take HRT.

Non-hormonal options

Moisturisers and lubricants used correctly help a great deal, and are not the same thing as one another. What to use and what to avoid.

Review

Treatment usually takes a few weeks to work, so there is a follow-up point rather than a prescription and silence.

If anything needs adding on the day

If a test is needed during your appointment it is £80 — cervical cytology, chlamydia and gonorrhoea, a urine culture, or a high vaginal swab. The full seven-infection STI panel is £220.
