Dyspareunia is one of the most under-reported symptoms in women’s health — many women live with it for years before mentioning it. The causes range from skin conditions and dryness to endometriosis and pelvic muscle spasm, and treatment follows the cause.

Where the pain is, when it started, and whether it is at entry or deep — discussed at your pace, in plain language.
Superficial or deep
Entry pain points to skin conditions, dryness, infection or muscle spasm; deep pain points to endometriosis, adenomyosis or pelvic disease. The distinction guides everything.
Examination only with consent
Gentle, explained first, stopped whenever you say — and skipped where the history already tells the story.
Ultrasound where needed
For deep pain, pelvic ultrasound looks for endometriosis, adenomyosis and ovarian causes in the same visit.
Treatment
Local treatments, hormonal options, pelvic physiotherapy referral, or treatment of the underlying condition — a plan that treats the cause, not just the symptom.
Urgent referral
If findings suggest anything requiring urgent specialist care, referral is arranged the same day.

Questions about painful sex
Is painful sex normal?
No — common, but not normal. Occasional discomfort happens; persistent pain has a cause worth finding.
What causes pain at entry?
Commonly vaginal dryness, thrush or other infection, skin conditions such as lichen sclerosus, or vaginismus — involuntary muscle tightening that is very treatable.
What causes deep pain?
Endometriosis, adenomyosis, ovarian cysts and pelvic infection are the usual suspects — most visible on examination and ultrasound.
I am embarrassed to talk about it
You will not be the first, or the hundredth. The consultation is unhurried and led by a female consultant; you set the pace and nothing happens without your consent.
Can menopause cause painful sex?
Very commonly — falling oestrogen thins vaginal tissue. This is one of the most treatable causes of all, and one of the most under-treated.
Should I also see my NHS GP?
